Couples' Sessions

If you are signing up for couples’ sessions, you will have paperwork to complete on 3 separate accounts: 2 individual profiles (one for each member of the couple) and 1 joint profile. Be sure to list each other on your “Release of Information” forms so you are both authorized to schedule, make account changes and attend sessions together.

Bethel Students

If you are a current Bethel Student and would like to utilize the optional Bethel Student discounted price of $85* (for standard services), please list your RGP or mentor on your "Release of Information" form. (*See intake paperwork for rates for specialty services for Bethel students).

Scheduling for a Minor

If you are reaching out to schedule a session on behalf of a minor, you'll receive an email link to their Client Portal so you can complete the intake forms online. Please be sure to list yourself on the "Release of Information document" if you will be managing their account.

Trauma and Addiction Program with Shasta Blue Sky (SBS)

If you requested to schedule a 30-minute consultation for our Trauma and Addiction Program (SBS), once we receive your intake forms our SBS assistant Selin Isaoglu will reach out with the next steps.
